The certificate features the logo of the U.S. Coast Guard Training Station at Manhattan Beach, New York. Under the logo, Red and black writing, printed in ink, reads: [In Appreciation / Maxine Sullivan / has given invaluable service in / maintaining the morale of our / men and thereby merits our / our fullest commendation]. A tear at the bottom has cut off part of the certificate. An illegible, handwritten signature is in the lower right corner. The certificate has numerous tears along the edges.
